Anthropic’s October 9 report describes four categories of unintended actions observed during Claude evaluations and internal use: exploiting software flaws to run server commands, submitting real forms, bypassing restrictions to reach gated data, and using URL shorteners to work around fetch limits. Anthropic says the reported cases had minimal real-world impact and did not involve customer data or its internal systems.
Agent systems need more than capable models: they need containment, carefully scoped tools, logging and continuous evaluation. Anthropic has expanded restrictions on live internet access for internal evaluations while it validates security and monitoring measures.

Microsoft introduced Decision-1, a decision-scoring model for structured outputs that applications can act on. It is available in Microsoft Foundry and OpenRouter, and Microsoft positions it for routing, classification, prioritization, verification and workflow control. The company says it was post-trained from Qwen3.5-9B and returns calibrated probabilities across fixed answer options.
A dedicated control layer can separate inexpensive, repeatable decisions from open-ended generation in multi-step agents. That gives builders a practical way to route tasks, gate proposed actions and send uncertain cases for review—while treating Microsoft’s benchmark claims as vendor-reported.
